When you join Thrive
Your annual fee covers the care that we provide. No additional copays, charges, or hidden fees. Membership includes:
Physician Visits
You’ll receive as many office visits, phone calls and virtual consultations with your Thrive physician as you need. If we refer you to a specialist, they will bill you/your insurance separately.
Evidence Based Care, Aligned With Your Goals
We provide comprehensive care based on the most up-to-date medical information and refer to top-notch specialists in the area.
Easier Scheduling & Longer Appointments
Each doctor has a limited number of patients in their panel, enabling them to provide personalized, in-depth healthcare that never feels rushed.
In-Office Testing & Services
Point of Care (POC) testing is included with your membership. Lab tests and vaccines are offered in-office and billed directly to your insurance without copay or reimbursement paperwork. See below for test details.
Connected Medical Records
With Epic as our electronic medical record system, your records will be connected to top Chicago-area health systems and specialists, allowing for seamless coordination of care.
Transparent Pricing
Our price model is simple and straightforward: one annual fee covers all the services we provide. Our one-time registration fee does not repeat. There are no additional copays or fees.
Our Services
Thrive membership ensures that there are no additional charges for office visits, virtual care, or any other communication with your doctor.
The following services are included in the annual fee:
Comprehensive Primary Care
- An annual comprehensive physical
- Timely visits to address illness or other acute needs
- In-person office visits as needed
- Virtual visits (when feasible and advisable as determined by Thrive) via the secure, HIPAA-compliant platform that we use in our practice
- Complete review of your medical history
- Management of chronic conditions
- Women’s Health Care including breast exams and pap smears
- Collaborative plan of care
- Medical care from a Thrive physician only (we do not utilize mid-level providers such as nurse practitioners or physician’s assistants)
- Support from skilled, caring and efficient medical assistants
In-Office Testing & Services
- Point of Care (POC) testing allowing for immediately available results (such as rapid strep, rapid flu, rapid covid, glucose, urinalysis)
- In-house lab draws and vaccines (billed directly to your insurance by the Northwestern Medicine lab or Vaxcare, respectively)
- ECGs
- Nebulizer treatments
- Body composition analysis on our InBody 970 scale
Coordination of Care
- Coordination with your specialists
- Coordination with your hospital team
- Access to our extensive referral network
Easy Communication
- Easy access to your physician by phone and text message
- Direct access to your physician by phone for after-hours emergencies
- Utilization of Northwestern Medicine’s EPIC electronic health record, allowing for seamless access to your records across local health systems and specialists
